Project Objective

The use of telemedicine technologies to implement a new model of comprehensive rehabilitation of patients with heart failure at their place of residence.

The programme will include, in six centres, 850 patients (randomized 1: 1; 425 in the study and control group) with heart failure I-III in the NYHA class , EF <40%, after an incident of hospitalization, in a stable clinical state. Patients from the study group will implement a 9-week rehabilitation program consisting of two stages: introductory stage 1 (a week at an in-patient ward), stage 2-essential (8 weeks of telerehabilitation at the place of residence). During the 24-month follow-up, the primary endpoint (days alive and out of hospital) will be assessed in both groups.

The project will enable the widespread use of early secondary prevention strategies in the population of people with heart failure. This will be achieved through the use of:

- new technology - telemedicine
